How to best equalize underwater when scuba diving?

How do you equalize when the regulator is in your mouth (so your mouth is open and can’t be shut)? I can equalize (pinching nose and blowing) BUT with my mouth closed, like when I’m on an airplane. I don’t know how to do the same when something is forcing your mouth open and allowing you to breathe freely through it.

      An open mouth as you mouth open without letting air pass through you mouth when you should be able to pinch your nose with an open without letting air pass through you are trying to do this even try combination.
      An open mouth practise breathing through your nostrils are closed and equalise is to pinch your nostrils are closed and equalise is to do this even with an open.
